To receive services from the Family Ties Project, eligible clients need to be enrolled in the project. The enrollment process includes informing the client about the Family Ties Project, its mission, goals and services offered, signing an informed consent, and assisting in the completion of a family genogram and demographic forms for each family member. These materials are used as part of our service delivery as well as project evaluation. Families who meet the project's eligibility criteria can be enrolled by case managers at Family Ties Project collaborating agencies, or by Michelle Palmer, Life Planning Specialist at the Family Ties Project. For further information on enrollment in the Family Ties Project, contact Michelle Palmer @ 202.547.3349 or by e-mail: mpalmer@familytiesproject.org The Family Ties Project's current enrollment forms can be downloaded in pdf format.
